Estimation of body fatness in obesity and partial lipodystrophy in relation to eligibility for bariatric surgery: What should be measured?

Efthymiadis Agathoklis , Vasan Senthil K , Tan Garry D , Karpe Fredrik

Introduction: Eligibility for bariatric surgery in the Unighted Kingdom is based on specific body mass index (BMI) thresholds and the presence of obesity-associated complications. Patients with partial lipodystrophy (LD) often have an abundance of metabolic and cardiovascular complications, but their abdominal adiposity does not always substantially increase BMI. The consequence is that patients with LD may be inappropriately excluded from obesity treatments when judged by BMI...

A case of ChAdOx1 vaccine-induced thrombocytopaenia and thrombosis syndrome leading to bilateral adrenal haemorrhage and adrenal insufficiency

Efthymiadis Agathoklis , Khan Dalia , Pavord Sue , Pal Aparna

Introduction: Vaccine-induced thrombosis and thrombocytopenia (VITT) after vaccination against SARS-CoV-2 with the adenoviral vector-based vaccines ChAdOx1 and Ad26.COV2.S has been associated with adrenal pathology, such as bilateral adrenal vein thrombosis, adrenal cortex haemorrhage and adrenal insufficiency in six percent of patients1.Case report: We report the case of a 23-year-old healthy woman who presented at eight days after ChAdOx1 va...

A case of ChAdOx1 vaccine-induced thrombocytopenia and thrombosis syndrome leading to bilateral adrenal haemorrhage and adrenal insufficiency

Efthymiadis Agathoklis , Khan Dalia , Pavord Sue , Pal Aparna

Case History: We report the case of a 23-year-old woman who developed adrenal insufficiency secondary to bilateral adrenal haemorrhage in the context of vaccine-induced thrombosis and thrombocytopenia (VITT). She presented with breathlessness and chest pain eight days after receiving her first dose of the adenoviral vector-based ChAdOx1 vaccine. Over the course of a week, she developed fulminant VITT. Her only comorbidity was obesity, with BMI of 35 kg/m2. <p class="abstex...

Coexistence of cranial diabetes insipidous and heart failure with reduced ejection fraction: a case report presenting challenges and unique therapeutic opportunities

Efthymiadis Agathoklis , H P Gamble James , Pal Aparna

History: We present the case of a 47-year-old man with heart failure (HF) and cranial diabetes insipidus (CDI) secondary to Langerhans-cell-histiocytosis. In the context of worsening HF with increasing shortness of breath and fluid retention, careful desmopressin dose reduction provided adequate aquaresis to restore euvolemia, obviating the need for usual diuretic treatments.Investigations: Echocardiography and a cardiac MRI revealed a non-dilated but se...

Comparative evaluation of diagnostic performance of the most commonly used screening tests for pathological hypercortisolism: A single centre analysis

Efthymiadis Agathoklis , Loo Helen , Shine Brian , Tomlinson Jeremy , Pal Aparna , Pofi Riccardo

Background: To date, there is no consensus as to the gold-standard screening test for diagnosing Cushing’s Syndrome (CS).Objectives: This study aimed to: a) compare the ability of late-night salivary cortisol (LNSC) against overnight dexamethasone suppression test (ONDT) and urinary free cortisol (UFC) as screening test for pathological hypercortisolism (PH); b) test the performance of those tests in diagnosing Cushing’s disease (CD) or mild au...

Post operative copeptin does not predict occurrence of the syndrome of inappropriate antidiuresis (SIAD) after pituitary transsphenoidal surgery

Efthymiadis Agathoklis , Rostom Hussam , James Tim , Shine Brian , Guha Nishan , Cudlip Simon , Christ-Crain Mirjam , Pal Aparna

Introduction: The incidence of the syndrome of inappropriate antidiuresis (SIAD) post pituitary surgery is estimated at 3-20% with the condition potentially leading to significant morbidity and prolonged admission. Copeptin, a surrogate marker for vasopressin activity, has been shown to be a diagnostic tool in the diagnosis of AVP deficiency (formerly known as diabetes insipidus) post pituitary surgery. There is limited data regarding the utility of copeptin in predicting SIAD...
